Report a Violation or Safety Issue

Use this form to report an ethics violation or serious safety issue. It will go directly to the the Ethics Director, a board-level role that investigates, implements interim protections when needed, and can hire an external investigator for serious allegations or when conflicts of interest are present.

Reports are handled by unconflicted board members with strict confidentiality and anti-retaliation protections. No one will ever be required to confront an accused person or participate in “repair” as a condition of safety.
